EKU is dedicated to the success of its full-time and part-time faculty. PROFS is designed to assist new faculty members in the transition to the Eastern community.

New Faculty Checklist*
What do I need for orientation?
  • Your Social Security card
  • A second form of identification (e.g., driver's license)
  • Documents required for the I9 form (see acceptable documents listed on form)
  • Completed forms (as described below); some require your signature
 
Have you...?
  • Completed the parking form with your vehicle information and printed it?
  • Activated your EKU email after reviewing the instructions?
  • Completed the employment forms (e.g., I9, W4, direct deposit), printed, and signed them?
  • Reviewed the Benefits Handbook, completed the portion(s) of the forms that you can complete in advance, and printed the forms? (Full-Time Only)
  • Completed the Campus Recreation Fitness & Wellness Center Membership and Payroll deductions forms and printed them? (if desired) (Full-Time Only)
  • Completed the meal plan form for faculty/staff after reviewing the dining brochure and printed it? (if desired) (Full-Time Only)
  • Logged onto www.HealthyYouatEKU.com and completed the Online Wellness Assessment (if desired) (Full-Time Only)

We appreciate you taking the time to assist with future orientation programs. Please do not complete this survey until the conclusion of all orientation activities.
